It is important to <strong>read<\/strong> <strong>questions 4 and 5 exactly as written<\/strong> when speaking with the insurance representative.&nbsp;<\/p>\n\n\n\n<ol class=\"wp-block-list\"><li>Insurance company verification and plan type (e.g., HMO, PPO)\u00a0\u00a0<\/li><li>Name of member services representative that spoke to you\u00a0\u00a0<\/li><li>Phone number, date and time of conversation\u00a0\u00a0<\/li><li>Does my policy offer coverage for hearing aids as either a medical benefit or as durable medical equipment?
